Player Information to Bring to Registration
- Players height, weight and birth date
- Name of Medical Insurance Co.
- School Name
- Home and work telephone numbers of BOTH parents
- Medical Doctor's name and telephone number
- Name, address and phone of an emergency contact person.
- New players need to show a copy of birth certificate or equivalent. |
| Every new player in Tustin, Region 96 (playing U8 and above),
and all players who have either not participated within the last 12 months or do not have a
